Job Summary
● Provides a range of social services to include assessment of individual/family needs, referral, crisis intervention and long-term stability.
General Accountabilities
● Conducts individual, couple and family assessment and diagnostic sessions.
● Create individualized case plans for each individual/family including SMART goals, monthly budgets, and ecomap.
● Provides referral, short term counseling and crisis intervention services as appropriate.
● Ability to move from case management to coaching strategies.
● Submits reports to government agencies as mandated by law.
● Consults with administrators regarding problems presented by staff related to psychosocial factors.
● Reviews and evaluates client progress and establishes follow-up contact.
● Determines evaluation methodology and establishes appropriate clinical techniques.
● Reports and analyzes results.
● Maintains confidential client records.
● Communicates program priorities and performance standards and assesses operations using these criteria.
● Plans and conducts quality assurance reviews and recommends changes as appropriate.
● Develops and maintains automated or manual systems and procedures to facilitate program operations.
● Maintain detailed documentation on program database within seven days of client contact.
● Assists in development of prevention, intervention, referral and outreach services
● Coordinate treatment plan with ELI staff weekly
● Working knowledge of social theories and practices
● Maintain updated licensure (LMSW or LICSW)

Job Qualifications
● Education: Degree in Social Work (BSW or MSW)
● Experience: 3-5 years of related experience; or equivalent combination of education and experience
● Willing to submit to background checks.

Salary Range
● 45,000-50,000 based on education, experience and certifications
